Hanoi - frequent buses from the bus station or flag down on Highway 1. Ninh Binh - a good base for visiting Tam Coc or Cuc Phuong National Park. Takes just over an hour. 40,000 VND. Sam Neua - daily bus to Laos, 180,000 kip or 500,000 dong. Departs at 8 am. Bus is frequently loaded heavily with cargo and many more people than seats and in this case moves very slowly and you may need to get out to walk up the steepest hills on foot. Scheduled to take 8 hours but in March 2014 took 16 hours. There is a also a 7 am minibus that leaves as far as the Na Meo border. However, you'd be on your own once you arrive at the border as only the 8 am goes all the way to Vieng Xai and Sam Neua.
Update: As of June 2015, the Sam Neua trip took 11-12 hours. The road is under construction so its quite dusty but is in fairly good condition. 500,000 dong is accepted on the bus after the trip starts. See the Laos page for visa requirements and prices based on nationality. Make sure you can pay in USD (cash only) as there isn't an ATM at the Nam Soi/Na Meo border. It is unknown if you can pay visa fees in VND or Lao Kip at the Nam Soi/Na Meo border. In June 2015, a form fee ($1 USD) was charged as I needed a visa-on-arrival application/form at the border.
